So what should you do? How should you practice?


First, thoroughly learn the chord progression to the song you are working on. it is very dificult to try to approach the next chord tone if you are not 100% sure what it is. transpose each progression, and play them in every key- by ear optimally, or write them out if you need to. I use a reference sheet to insure I hit every combination of movement within a chord, and very common motions between chords.
